Locked out by multi-factor authentication

What to do when the approval prompt never arrives or your phone changed.

Most multi-factor lockouts come from a replaced phone, a reinstalled Authenticator app, or notifications turned off. Try these before requesting a reset.

  1. Open Microsoft Authenticator directly and read the 6-digit code instead of waiting for the push notification.
  2. Check that the phone has signal or Wi-Fi and that the device clock is set to automatic — a wrong clock invalidates every code.
  3. On the sign-in screen choose 'Use a different verification option' to try a text message or call to your registered number.
  4. If the phone was replaced or lost, open a Microsoft 365 ticket marked high priority so a technician can reset your registration.

Never approve a prompt you did not trigger. If approvals keep arriving unprompted, deny them and open an urgent ticket — someone has your password.
